Position Summary

The Licensed Practical/Vocational Nurse (LPN/LVN) provides direct nursing care to residents and supervises nursing assistants in accordance with current federal, state, and local regulations. This role ensures high standards of care and supports the nursing team under the direction of the Director of Nursing or Nurse Supervisor.

Essential Duties and Responsibilities

Administrative Functions

  • Supervise nursing assistants and coordinate shift duties.
  • Maintain compliance with facility policies and nursing regulations.
  • Participate in quality assurance efforts, resident admissions, transfers, and discharges.
  • Document care activities and incident reports accurately.

Charting and Documentation

  • Complete and maintain required charts, records, and care documentation.
  • Transcribe and implement physician orders.
  • Ensure thorough and timely charting of resident care.

Medication Administration

  • Prepare and administer medications as prescribed.
  • Ensure safe and accurate medication handling.
  • Report discrepancies and monitor inventory.

Nursing Care

  • Provide and oversee resident care based on care plans.
  • Respond to changes in condition and coordinate with physicians and families.
  • Perform clinical procedures such as catheter care, wound care, and vital signs monitoring.

Personnel and Team Support

  • Provide leadership and training to nursing staff.
  • Participate in performance evaluations and staff development.
  • Foster a positive and supportive work environment.

Safety and Infection Control

  • Promote and maintain clean, safe, and sanitary conditions.
  • Follow infection control and safety protocols.
  • Ensure availability and use of protective equipment.

Knowledge, Skills, and Abilities

  • Strong clinical and leadership skills.
  • Knowledge of nursing procedures and care standards.
  • Ability to make sound decisions and work collaboratively.
  • Compassionate, organized, and detail-oriented.

Education & Experience

  • Graduate of an accredited LPN/LVN program.
  • Active, unencumbered LPN/LVN license in this state.
  • Current CPR certification; PCC experience preferred.
  • Ongoing compliance with CE requirements and licensure standards.
